The Crucial Role of Responsibility in Construction

Six years on from the Grenfell Tower fire and there is still so much to be said. It is hard to believe just how many things went wrong. Not only on the night of the fire, but the decades before, and the time that followed. The misconduct in practice and errors of judgement from all parties turned what should have been a ‘normal flat fire’ into one of the deadliest modern-day residential fires this country has seen. After closely following the Grenfell Tower public inquiry, one theme has become painfully clear: the crucial role of responsibility, and the devastating consequences of its absence. Frequent use of terms like competency, reliability, and responsibility can sometimes desensitise us to their true significance. However, these values are important in ensuring fire safety and, ultimately, in saving lives. This article will explore the lack of responsibility, touching on examples from the night of the fire and the periods before and after the fire. It is important to note that these are only a few examples pulled from Show Me the Bodies by Peter Apps, a full breakdown of the Inquiry’s evidence can be found here. What went wrong? The night of the fire The external cladding was found to be the primary cause of the fire’s spread. ACM (aluminium composite material) cladding is made from a highly combustible polyethylene polymer filler which when exposed to fire, buckles, and exposes the internal material to flames. It was installed onto the existing structure, leaving a gap between the façade and the building’s structure. This caused what is known as a ‘chimney effect’ and enabled the fire to continue its path of destruction up the tower’s exterior. While this was taking place, inside the tower was also becoming unmanageable. When the correct passive fire protection products are used and installed correctly, this effectively slows the spread of fire and smoke, allowing time for evacuation and the local fire service to arrive on the scene. But this did not happen at Grenfell. The compartmentation of the building was broken; the inquiry found that “forty-three front doors in the tower had no self-closers installed and thirty-four were not working” (p. 97). The knock-on effect this had was immeasurable, with two-thirds of fire doors from the fourth floor upwards not closing by themselves when residents evacuated. The fire that should have been successfully contained to one compartment of the building was now out of control, quickly finding access to the outer side of the building and spreading to other areas of the tower. Meanwhile, on the ground, there was no contingency plan to evacuate Grenfell. The London Fire Brigade (LFB) had a lack of coordination and communication, and with no training on what to do if the stay-put policy failed, this left trapped residents in trouble. It was found that only three phone calls from trapped residents resulted in a fully successful rescue mission and that if the stay-put policy was abandoned, trapped residents could have fled and survived up until 2:45 am. Since the fire, the LFB’s response to Grenfell has been strongly criticised. However, there are opposing views that argue that ‘stay put’ is a recognised policy. If Michael Dowden, the initial incident commander, did improvise and order an evacuation where 30 people died, “he’d have been the firefighter that killed 30 people” (p. 57). So, it begs the question: was it always going to be a lose-lose situation? Rewind to before the fire There is a decade-long picture that points to why the fire on June 14th was able to escalate, and how such a dangerous building came to be. This picture exposed the incompetence and misconduct within this country’s government and construction industry. Let’s start with the decades of deregulation. In April 2011, a red tape challenge was declared because excessive fire safety regulation was seen to be “burdening businesses, hurting our economy and damaging our society” (p. 64). Eric Pickles, who served as secretary of state at the then Department for Communities and Local Government, promised to “save businesses 25.4m per year by cutting [fire safety] rules” (p. 66). The mindset that fire safety wasn’t important enough to be made a priority meant that civil servants failed to drive through significant amendments to the building regulations. Brian Martin, previously responsible for building regulations and guidance on fire safety, had been asked to publish an FAQ explaining that ACM cladding was banned in 2014. This never happened. As we have seen time and time again, relaxed building standards and mindsets almost always end in disaster. Meanwhile, manufacturers were acting with no regard for fire safety: “The fire safety risk of these [ACM] panels were discovered in 2004. They could have been taken off the market then. But no one was willing to risk corporate profits for basic safety” (p. 106). Fire tests were rigged, information was withheld, and marketing continued to present ACM cladding as the go-to solution to an industry ignorant to the ever-growing fire safety crisis. Like the LFB, there was a lack of knowledge among the architects, builders, and contractors involved in the 2015-16 refurbishment of the tower. Between these trades, the decision was taken to use ACM cladding to ‘cut costs’. It was also revealed that “if a job fell in a certain inspector’s postcode, it was theirs – regardless of their skill and experience” (p. 179). Incompetent people were being appointed to work on the Grenfell Tower, and responsibility was given no second thought. Fast forward to after the fire After 400 days, the Grenfell Tower inquiry came to an end. Richard Millett KC, counsel to the inquiry, in his final statement, said: “Each and every one of the deaths that occurred in Grenfell Tower, on the 14 June 2017 was avoidable.” He argued that the organisations involved in the refurbishment of the tower were “spinning a web of blame” and not taking any responsibility for their actions: Arconic, the company that made the panels and knew they were dangerous

The foam concentrate is usable at 3% at low and medium expansion. ECOPOL 3N can be used on gentle application, as well as on direct application on flammable liquid. This concentrate is also protected against frost. Its liquid formulation makes it easy to use with most of foam equipment. It does not induce foam equipment modification when transitioning to FFF. ECOPOL 3N is an ecological solution. It does not contain any fluorinated derivatives (PFAS chemicals including PFOS or PFOA) or PBTs (Persistent, Bioaccumulative and Toxic substances) added intentionally. ECOPOL 3N is easily biodegradable, without impact on the environment. Industrial companies and fire departments are confident using it on tank fire and structural fires. Specialist wildfire units to come to the UK

Climate change has been ravaging the earth for decades, but in recent years we have seen a huge increase in threat from wildfires. Common countries such as the US and Australia continue to burn by the hectare every year, but now wildfire hotspots are increasing all over the world. With the recent continuous spread across areas of Canada, causing evacuations, destruction and even smoke protection alerts stretching as far as the US, countries around the world are gearing up for another wildfire season. In the UK, a country notorious for its grey, cloudy and rainy weather, fire and rescue services are now forced to adapt and learn from the world’s worst wildfire hotspots as they prepare for the dry and hot Summer ahead. Last year’s heatwaves saw thousands of fires break out, one even destroying homes in London, and this year is expected to see the same risk with hundreds of grass fires across England and Scotland in recent weeks. More fire crews are now training in skills from southern Europe, like Spain, and the US to better understand the tactics and equipment necessary to battle wildfires in a more efficient way. According to the BBC, fire chiefs are particularly looking to expand the number of specialist teams trained in “burn suppression” techniques – the deliberate burning of land to keep a fire contained. The BBC also reported that Chief Fire Officer Paul Hedley, wildfire lead for the National Fire Chiefs Council (NFCC), said more units – including those in services that straddle urban and rural areas – would be trained in new ways of wildfire fighting as the risk and threat of major wildfires is clearly growing in the UK. Fire services from around the world, although have a competitive edge against each other, have always learnt beneficial training techniques to fight new modern dangers from each other, whether this be highrise buildings and the growing built landscape, sustainable efforts and nozzle changes, contamination reduction, and now, wildfire training and techniques. Cradlepoint enhances public safety

Cradlepoint, in cloud-delivered LTE and 5G wireless solutions, announced that Avon Fire and Rescue Services (Avon FRS) will be using Cradlepoint routers, powered by NetCloud to provide failover support to ensure unfailing and continuous connectivity across the Service’s 21 stations. Avon Fire and Rescue Service provides prevention, protection, and response services across the former Avon area, which spans 512 square miles, including an international airport, five motorways, four universities, and 450,000 homes. As a result, Avon FRS requires the best connectivity to ensure unfailing communications, allowing its firefighters to undertake their duties and protect the lives of Avon’s 1.1 million population. Avon FRS received notice from its previous provider that its secondary connectivity bearer would be withdrawn at the end of March 2022. Following this, Avon FRS engaged with Telent, Cradlepoint’s channel partner; and together quickly deployed Cradlepoint’s NetCloud Service and wireless edge routers to Avon FRS’ 21 stations to provide a futureproofed, resilient solution. The deployment of the Cradlepoint technology, formed part of a larger project delivered by Avon FRS and Telent to provide a complete technology refresh to Avon FRS’ station end mobilisation system. Through the deployment, Avon FRS gained further insight into its network, leveraging Cradlepoint’s cloud-based NetCloud platform, affording them the ability to monitor data accurately. Unlike the previous system, Netcloud proactively flagged when connectivity failed, and allowed engineers to address any network issue remotely, significantly reducing downtime. This increases the day-to-day ability of each firefighter to carry out their duties effectively, not having to worry about losing contact with their home station. Since the delivery of the solution, Avon FRS has benefitted from more reliable and secure networks through improved failover connections. Representatives from Portland Fire and Rescue and Pierce Manufacturing will welcome the fire department’s new Pierce Volterra electric pumper into service with a “push-in” ceremony open to the public. The revolutionary fire apparatus was introduced in 2021 as the first of its kind in service in North America. Portland Fire and Rescue’s new electric fire truck will support municipal environmental sustainability initiatives with its groundbreaking green technology. The traditional “push-in” ceremony is a fire service tradition dating back to the 1800s. Fire apparatus were not easily backed into fire stations with horses alone, so after a call, firefighters would have to disconnect the fire equipment from the horses and push the fire apparatus back into the bays. Continuing the tradition, when a new fire engine is purchased, fire departments will often hold an event to ceremonially push the new truck into the bay. In-person attendees will have the opportunity to get an up-close view of the electric fire apparatus, inside the cab, all technology elements, and more. ROCKWOOL insulates new Homes in Adamstown, Co. Dublin

Hugh McGreevy takes pride in delivering energy efficient homes, constructing to Building Energy Rating (BER) A rating, and to a quality that stands the test of time. ROCKWOOL was invited to present the benefits of NyRock technology, and how the latest product, NyRock Cavity Slab 032 could help deliver the required fire and thermal performance. Hallwell by Hugh McGreevy & Sons Ltd is a development of 100 new homes being constructed as part of a larger residential area in Adamstown, Co. Dublin, Ireland. Included in the scheme are duplex homes and apartments in three- and four-storey blocks due to be completed in mid-2023. The homes, designed by Hugh McGreevy’s in-house team, were planned with a masonry cavity wall construction and a preference for stone wool insulation due to its non-combustible properties. Working with the developer, ROCKWOOL conducted all U-value calculations for the project.  These demonstrated that NyRock Cavity Slab 032 would achieve the target U-value of 0.18 W/m2K within the 200mm cavity space. This led to the specification of 14,000 square metres of 160mm NyRock Cavity Slab 032 in a partial fill application. To satisfy fire regulations in cavity applications, 9,000 metres of ROCKWOOL TCB Cavity Barrier was also specified. In addition to its combined thermal and fire performance, the high density semi rigid composition of NyRock Cavity Slab 032, is proven to perform for the lifetime of the building,[1]  supporting Hugh McGreevy & Sons’ focus on quality and longevity. “ROCKWOOL was our first choice for non-combustible insulation due to their reputation in the market and our experience with their stone wool products on previous projects,” said Hugh McGreevy, Director of the company. “NyRock Cavity Slab 032 has enabled us to achieve the same U-values using less material, which was an additional bonus.” “Our installers found NyRock Cavity Slab 032 very easy to install,” continues McGreevy. “The insulation provided no problem for the bricklayers, even in adverse weather conditions with some heavy rain. The window fitters found it equally easy to work with when they had to cut to fit it around windows. “ROCKWOOL NyRock Cavity Slab 032 has BBA certification, is easy to source, and has an experienced technical team able to answer any questions very quickly. This ultimately made it easy for us to adopt NyRock for this development.” With ROCKWOOL NyRock Cavity Slab 032, the new homes in the Hallwell development will meet the required U-value of 0.18 W/m2K. Alongside its improved thermal efficiency, NyRock Cavity Slab 032 still delivers all the additional benefits of stone wool insulation including the ability to be recycled indefinitely, and acoustic and fire performance. ASFP releases new passive fire protection guidance

The Association for Specialist Fire Protection (ASFP) has published three new Advisory Notes and updated another. The documents cover a range of topics related to firestopping, penetration seals and smoke seals. Advisory Note (AN) 13 covers the testing of partial penetrations, for example, socket boxes, which are installed into dry wall systems. Firestopping products are made to help maintain the performance of a wall where such electrical boxes have been added. A CEN standard is currently being drafted to cover the testing of these products. However, in the interim period AN 13 suggests that the firestopping of these products is undertaken using an ad-hoc approach, based on testing to BS EN1364-1: Fire resistance tests for non-loadbearing elements. Walls Advisory Note 30 covers the transition from CE marking to the UKCA mark. It explains how all of the different passive fire protection product families are affected, describing the likely next steps in the process. It aims to help manufacturers and specifiers alike to understand what will be required during the transition period. Advisory Note 31 covers smoke seals used in conjunction with active fire curtains. AN 31 was produced in response to false claims that manufacturers used draught excluders to prevent the passage of cold smoke and that these would be of limited use in a fire resistance test scenario. The document explains that the performance smoke seals used are the same technology as might be used in smoke leakage-rated fire-resisting doors. Where smoke seals are added for a leakage test, they also have to be present for the corresponding fire test. Advisory Note 15 has been updated. This explains the interchange of flexible wall systems for service penetration seal systems. When first published, this note provided pictorial clarification of a long-winded statement in BS EN1366-3: 2009: Fire resistance tests for service installations. Penetration seals. This standard has been amended, and some of the scenarios permitted in the 2009 version are not now reproduced in the 2021 version. The drawings in AN15 have been amended to bring them in line with BS EN 1366-3: 2021. ASFP Advisory Notes are available to download for free from the ASFP Publications library. LFB welcomes latest action on e-scooters

London Fire Brigade has welcomed action by train companies in response to the fire risk of lithium batteries in electric powered personal vehicles on their service. From June 1, e-scooters, e-unicycles, e-skateboards and hoverboards will be banned on trains and at stations run by several train companies including Govia Thameslink Railway and Southeastern. Deputy Commissioner Dom Ellis said: “We are very pleased that further train operators have taken action in response to the risk e-scooters can pose and are trying to mitigate these risks. “While these are positive steps, our latest data shows that firefighters have attended 53 e-bike fires in London already in 2023, which is four times as many incidents as those involving e-scooters. Given e-bikes are not included in the ban, we are also encouraging operators to consider whether they have adequate safety measures in place should an e-bike fire happen on their service.” Following an increase in lithium battery fires, in particular e-bikes and scooters the Brigade has been running its #ChargeSafe campaign in a bid to raise awareness of the issue and promote how to safely use products with these batteries. IFE celebrates contributions of its 400 volunteers

The Institution of Fire Engineers (IFE), marks the 39th year of Volunteers’ Week (1-7 June) by recognising and celebrating its volunteering cohort in shaping a world safer from fire Throughout the week, the IFE will release interviews with volunteers from across the globe, providing insight into the incredible work they do each day in driving advances across the fire sector. At a team of over 400, IFE volunteers are a vital resource and support the institution in every aspect of running the charity, from developing CPD and evolving best practices and research, to expanding the ability to carry out examinations. Over the last 12 months, the Membership Application Assessment Panel and IFE Registrants Group have processed over 1,150 membership and registration applications, while its exam volunteers have written 55 examinations. The IFE continues to explore specialist subject areas within fire engineering through its group of 10 Special Interest Groups (SIGs). These include electric vehicles and emerging technologies to fire safety within heritage sites, often influencing government legislation in the process. Commenting on Volunteers’ Week, IFE CEO, Steve Hamm, said: “For over 100 years, the IFE has benefited from the incredible knowledge of its volunteering cohort.
